  1. Have credit card ready and click the blue button below to use the Online Form to submit your form electronically and remit any funds due via credit card. Please note that this membership form is intended for use on desktops, laptops, and tablets. You should receive an email acknowledging receipt of your membership form and payment shortly after your online submission. If you do not, please click to contact our Forms Expert.

  2. To complete your membership form manually, click the red button below to open the fillable PDF form, then save it if you wish to type in the information or print it to fill it out manually by PRINTING NEATLY and LEGIBLY. Bring the form and payment (check, cash, credit card) to the Membership table at the next Guild meeting or mail the form and a check to the address listed on the form.
